Chairman's survey

Champagne survey

The survey has now officially closed. The prize draw was made at the Christmas meeting on 29 December 2005.

However we are still interested in hearing members' views about the branch and how we could serve you better. The form is still active and your comments and suggestions will be logged and studied.

The input received up till November 29 has been tabulated. We now have to sift through your comments, try to reconcile those with contrasting opinions, and feed the results into our planning for 2006/7.

champagneHelp us understand how well our branch is meeting your expectations: fill in this questionnaire for a chance to win a bottle of champagne.

We would welcome your comments about our meeting programme and other branch activities.
In case you missed the 2005 AGM, or didn't have time to complete the form on that day, this is an updated version of the questionnaire which was handed out then. Please fill it out with your comments or ideas for future branch events.
Every branch member who completes this form by early on 29 November 2005 will be entered into the draw for a bottle of champagne, to be handed out at the Christmas Lecture on 29 November.
Sorry, committee members are not eligible

Note: change of closing date because the Christmas meeting has been brought forward.

If you have second thoughts and want to come back and submit more comments, that's fine, but each name only goes into the draw once.

Questions marked with *M* are mandatory

Apologies

The input checking for this form was causing confusion by treating the "Other branches" question as mandatory even though it was not marked as such.
The webmaster apologises and thanks those branch members who reported the problem. It should be OK now but please let us know if you find any more problems.
